As effectiveness of safety reporting systems are underpinned by a positive reporting culture, how can Administrations/Organizations foster a culture that encourages reporting and ensures effectiveness of these systems. While systems and sources require protection, implementation, and follow-through, is there a one-size-fit-all solution, or are there different means of effective implementations? The Panel will share successful safety reporting systems and practices that support positive reporting culture and drive safety improvements, and challenges.
